If your insurance requires a referral, then contact Christine at 503-452-0915, ext. #38. If you have Providence ONLY, then contact Lisa at 503-452-0915, ext. #15. 

If you have not been seen by your primary care physician regarding the issue for which you are seeking a referral, you may be asked to come in for an evaluation prior to being referred to a specialist.
